Abstract: 

Introduction. Urinary tract infection is the most common serious bacterial infection in children. The aim of this study was to compare the value of different laboratory and imaging techniques in detecting renal involvement in acute pyelonephritis.Materials and Methods. In a cross-sectional study of patients 1 month to 14 years of age diagnosed with urinary tract infection were examined with systemic inflammatory markers, renal ultrasonography, voiding cystourethrography (VCUG), and TECHNETIUM Tc 99M dimercaptosuccinic acid (99MTc-DMSA) renal scintigraphy. A total of 1467 pediatric patients were eligible for treatment of pyelonephritis. Evaluations included a complete blood count, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), urinalysis, urine culture, and measurement of serum blood urea nitrogen and creatinine levels. Results. The results of 99MTc-DMSA scans were normal in 20.2%, mild decreased cortical uptake in 45.0%, moderate decreased cortical uptake in 12.3%, severe decreased cortical uptake in 12.0%, and decreased cortical function plus irregularity or scar formation in 10.5%. Voiding cystourethrography showed vesicoureteral reflux in 25.9%. The sensitivity of 99MTc-DMSA for prediction of vesicoureteral reflux was 84.1% with a negative predictive value of 80.6%. Significant differences were found in the level of blood leukocyte count (P = .03), urine leukocyte count (P = .003), ESR (P = .008), and age (P = .04) between patients with normal and abnormal 99MTc-DMSA scan results.Conclusions. We found that in patient with clinical signs of pyelonephritis, 99MTc-DMSA renal scintigraphy can detect pyelonephritis more accurately than the other inflammatory and imaging tests.

Abstract: 

متاستاز استخوان یکی از عوارض مهم و ناتوان کننده بیماریهای بدخیم به شمار می رود از آنجائیکه طول عمر نسبی بیماران مبتلا به متاستاز استخوان عموما زیاد است، تشخیص سریع و به موقع این ضایعات می تواند سبب بهبود کمی و کیفی زندگی بیمار شده و از پیشرفت کنترل نشده بیماری و بروز عوامل ناتوان کننده پیشگیری کند.در این مطالعه 22 بیمار مبتلا به سرطان بدخیم اولیه (با تشخیص های مختلف) و علائم و نشانه های متاستاز در دو نوبت مورد ارزیابی قرار گرفتند. بیماران انتخاب شده یکی از پاتولوژیهای زیر را داشته اند:Small Round Cell Tumor، کانسر پستان، کانسر پروستات، کانسر نازوفارنکس و مولتیپل میلوم. ابتدا اسکن استخوان به وسیله 99M Tc-MDP به عمل می آمد و بلافاصله یک هفته بعد اسکن کامل بدن با 99M Tc-MIBI تکرار می شد (در این فاصله هیچ درمانی برای بیماران انجام نمی شد). تعداد مناطق جذب و نیز شدت جذب ضایعات در اسکن 99M Tc-MIBI به صورت کیفی و نیز کمی ارزیابی شده و با اسکن استخوان مورد مقایسه قرار می گرفت. بعد از مقایسه نتایج، مشخص گردید اسکن تمام بدن با 99M Tc-MIBI تنها در 14 نفر از 22 نفر مثبت شده، و در این بیماران در مجموع 44.4% تعداد کل ضایعات استخوانی کشف شده در استخوان با 99M Tc-MDP را نشان داده است (24 ضایعه از 54 ضایعه) که میانگین شدت جذب آن نیز حدود +1.5 (خفیف تا متوسط) بود ولی در عوض در 7 تن از بیماران که دارای اسکن مثبت بودند بعضی از ضایعات پنهان بافت نرم را نیز عمدتا در نواحی گره های لنفاوی با میانگین شدت 20 (متوسط) به تصویر کشید. بنابراین به نظر می رسد در بررسی متاستازهای استخوانی، اسکن استخوان با 99M Tc-MDP در مقایسه با اسکن 99M Tc-MIBI  یافته های بیشتری را نشان می دهد، در حالیکه اهمیت اسکن 99M Tc-MIBI عمدتا در کشف ضایعات بافت نرم است.

Abstract: 

Introduction: The aim of this study was to evaluate the accuracy of TECHNETIUM Tc 99M dimercaptosuccinic acid (99MTc-DMSA) renal scintigraphy in the diagnosis of urinary tract infection (UTI) in children with suspected infection but with a negative urine culture. Materials and Methods: The records of all children with suspected or definite diagnosis of UTI presented during a 2-year period were reviewed in this study. Abnormal findings on renal scintigraphy, voiding cystourethrography (VCUG), and ultrasonography were evaluated and compared between the patients with the definite diagnosis of UTI and those with suspected UTI and negative urine cultures. Results: Of 210 patients, 86 had a definite diagnosis of UTI (group 1) and 124 had suspected UTI without a positive culture (group 2). Abnormal findings on DMSA scans were seen in 76 patients (88.4%) in group 1 and 84 (67.7%) in group 2. Vesicoureteral reflux was detected by VCUG in 50% and 32.3% of the patients in groups 1 and 2, respectively. In group 2, vesicoureteral reflux was seen in 40.5% of the patients with abnormal DMSA scan. Ultrasonography findings were abnormal in 51.3% and 39.8% of the patients with abnormal DMSA scan findings in groups 1 and 2, respectively. Conclusion: According to our findings, in children with a negative urine culture and abnormal urinalysis, 99MTc-DMSA renal scintigraphy is helpful in diagnosing UTI and vesicoureteral reflux; we recommend VCUG when DMSA scan supports UTI despite a negative urine culture and a normal ultrasongraphy.

Abstract: 

Background: Breast cancer is the first fatal disease in women and the second leading cause of death in humans, after heart disease. In approximately 20% of these patients, breast cancer cells have a higher-than-normal level of a protein called human epidermal growth factor receptor 2 (HER2) on their surface. These cancers, called HER2-positive, tend to grow and spread faster than other breast cancers. Objectives: This study aimed to evaluate the binding of the peptide LTVSPWY labeled with the HER2 receptor compared to the non-labeled peptide and simulate its docking. Methods: The LTVSPWY peptide was first labeled with the 99-radioactive TECHNETIUM (Tc-99M) and optimized for molecular docking with the HER2 receptor in HyperChem software. Then, the HEX 6 software was used to check how the compounds were connected to the active site of the HER2. Finally, data was analyzed by HEX, LIGPLOT, and other programs. Results: During docking in HEX, the labeled peptide showed a good link with the HER2 protein receptor. The energy of this kind of transplantation was higher than the nonlabeled peptide bond with the HER2 protein receptor. In fact, this combination had the lowest level of connection energy with the receptor, which reflects the success of docking. Conclusions: In the analysis of the post-docking data, we observed the ligand was connected to the HER2 receptor from 5 major sites at different intervals, indicating a suitable bond to the receptor ligand.

Abstract: 

Introduction: For renal scintigraphy, different radiopharmaceuticals like 99MTc-DMSA and 99MTc-EC can be used. Although these methods are accurate, some differences can be observed among them. These radiopharmaceuticals have different levels of reproducibility and repeatability. This study aimed to evaluate the level of inter and intra observer variability in 99MTc-DMSA scintigraphy. We also compared the renal function, measured with 99MTc-EC, with the one measured using 99MTc-DMSA scintigraphy to determine if 99MTc-EC can be used instead of 99MTc-DMSA in this regard.Methods: The sample volume consisted of 81 patients underwent both 99MTc-DMSA and 99MTc-EC in Imam Reza Health Center in 2008. These scans were interpreted by two observers. One of the observers interpreted the scans after one month for the second time. The data were analyzed with SPSS.Results: There was a close correlation between these two methods with respect to DRF (P = 0.51). Estimating intra observer variability showed close correlation between DMSA (r = 0.997) and EC (r = 0.996). The evaluation of inter observer variability also revealed high correlation between DMSA (r = 0.995) and EC (r = 0.996).Conclusion: Since the comparison between these two methods in measuring renal function showed the same results, 99MTc-EC can be used as a substitute for 99MTc-DMSA. In addition, Tc-99M EC scintigraphy can be a reliable single-modality study to evaluate renal cortical defects, perfusion, and drainage of the urinary system and indirect evidence of vesicoureteric reflux with the added advantage of low radiation exposure to the patient. Also good reproducibility and repeatability were reported according to this study.

Abstract: 

Objectives: 99MTc-MAG3 is a standard radiotracer for renal dynamic functional study. Despite its properties for clinical uses, it has numerous technical limitations. 99MTc-EC is also a tubular radiotracer for renal imaging, which has not been used worldwide. In this study, the use of 99MTc-EC and 99MTc-MAG3 for renal functional study were compared.Methods: Thirty five patients (20 male, 15 female; mean age of 34.63 ±10.69 years) were entered in the study. About 10 mCi of 99MTc-EC and 99MTc-MAG3 were administered in different days within 7 days intervals and serial images were obtained for 30 minutes. Serum creatinine and visual scintigraphic findings of all patients were within normal limits.Results: In this study, the renal uptake of 99MTc-EC was significantly higher than 99MTc-MAG3 (6.20% vs 4.39% of the injected dose), while the hepatic activity of 99MTc-EC was significantly lower (307 vs 439 mean pixel count, p<0.0001). Also the absolute values for some other quantitative parameters such as right and left kidney transit times and ERPF were different for each radiotracer. In spite of these differences most quantitative parameters (except for right kidney transit time, liver uptake and Tmax of both kidneys) showed good correlations for both agents.Conclusion: The calculated ERPF with 99MTc-EC as compared to 99MTc-MAG3 is in closer proximity to the true values. Also the use of 99MTc-EC compared to 99MTc-MAG3 in clinic due to simplicity of preparation, higher stability, higher renal uptake, and lower hepatobiliary activity is better. Therefore the use of 99MTc-EC as an appropriate substitute or ever preferred radiopharmaceutical for 99MTc-MAG3 radiopharmaceutical for renal function studies is recommended.

Abstract: 

Introduction: This article as the first clinical prospective study was conducted to use of Tc-IgG as compared with autologous Tc-RBC in gated blood pool ventriculography. Moreover, two objectives were followed: (a) assess the feasibility of this radiotracer in routine practice; (b) appraise its in vivo steadiness over time by measurement of organs activity ratios and the image quality.Methods: the study population included 12 patients who referred for evaluation of liver hemangioma or infection process. Six patients underwent gated planar blood pool (GPBP) acquisition using Tc-RBC and remaining 6 cases underwent GPBP with Tc-IgG. Additionally, to evaluate the activity ratios in different organs, ROIs were manually drawn over the LV, RV, right lung, left lung, aorta, liver and spleen on the static anterior datasets in 20 minute (early) and also delayed (180 minutes) post injection of Tc-RBC and Tc-IgG.Ratios of activity between the early and delayed images were compared for each ROI.Results: The mean time of acquisition following injection of each radiotracer (RBC; 75.83±11.75 minute Vs IgG; 80.00±7.09 minute) was not different [p value>0.05]. Observer 1 and 2 were classified good quality for both radiotracers in all participants. The mean ejection fraction (EF) of patients with Tc-RBC and Tc-IgG wan not significantly different (59.00±7.64 % vs.58.33±16.03 %) [P value>0.05]. The ratio of LV to crescent background in GBP was not statistically difference between two agents (RBC; 2.55±0.99 Vs IgG; 2.11±0.78) The mean activity of LV and RV to backgrounds ratios in patients with Tc-IgG was more (although nonsignificant) relative to cases radiolabeled with in vitro Tc-RBC on early and delayed views (P>0.05). In addition, the relevant constancy of biodistribution of both radiotracers over time was also compared in the organs activities between the early and delayed studies.Conclusion: This data showed that the use of Tc-IgG in cardiac blood pool studies might led to comparable images as Tc-RBC. Therefore, 99MTc-IgG, readily accessible and needing a single injection, might be an attractive alternative to Tc-RBC for the estimation of LV function, especially in subjects with poor vein quality or busy departments.
